This role is primarily remote, offering you the flexibility to serve clients from the comfort of your own space while maintaining the highest standards of care. As part of a hospice center, your practice may require sensitivity and specialized knowledge related to end-of-life issues, grief, and bereavement. • Conduct comprehensive assessments of patients and their families to determine social, emotional, and environmental needs while developing tailor-made treatment plans.
• Provide individual and family counseling sessions to address issues related to terminal illness, grief, and loss, utilizing therapeutic techniques that resonate with your clinical approach.
• Coordinate care with interdisciplinary teams, ensuring comprehensive support and continuity of services tailored to the client’s specific needs and wishes.
• Utilize our user-friendly EHR portal for client management, scheduling, and documentation, with live support available when needed.
• Participate in regular training and peer support groups to share insights and enhance your therapeutic skills, contributing to a culture of continuous improvement.
• Manage your scheduling and client interactions independently, allowing you to create a balanced workflow that suits your lifestyle and professional goals.

Your primary focus will be working with individuals and families grappling with terminal illnesses. This role demands a high level of empathy and compassion as you assist clients in navigating complex emotional landscapes during their most challenging times. You will facilitate difficult conversations about treatment options, end-of-life care, and legacy-building while providing practical resources for pain management and emotional comfort.

• Must hold an active licensure as a Licensed Clinical Social Worker (LCSW) in the United States.
• A Bachelor's degree in Social Work or a related field is required.
• Possess malpractice insurance to ensure compliance and protection while delivering clinical services.
• Demonstrated ability to provide compassionate care and support in a hospice or similar setting.

• 1-3+ years of clinical experience, specifically in hospice, palliative care, or end-of-life counseling.
• Familiarity with administrative tasks such as scheduling, documentation, and insurance credentialing.
• Experience utilizing telehealth platforms to conduct virtual appointments, offering flexibility to clients.
