  • Deliver brief, focused therapeutic interventions for patients requiring mental health services, leveraging various techniques and methodologies tailored to individual needs.
  • Advise on patient referrals to specialty mental health facilities when necessary, ensuring seamless transitions in care.
  • Initiate follow-up communications to monitor patient progress and adjust treatment approaches based on their evolving needs.
  • Help patients develop and implement relapse prevention strategies while supporting their journey toward stable functioning.
  • Identify at-risk patients and collaborate with other providers to develop preventive care plans, aiming to mitigate worsening mental health conditions.
  • Coordinate care delivery related to behavioral health, connecting clients with other treatment providers inside and outside of our primary care setting, with their consent.
  • Document patient progress in a detailed treatment chart, maintaining adherence to regulatory and organizational standards.
  • Collaborate actively with primary care providers to create comprehensive treatment plans that align with patient needs and progress.
  • Educate patients on self-management skills and lifestyle changes that promote healthier choices and positive outcomes.
  • Consult with management and team members on behavioral health services, contributing insights for program evaluation and outcome enhancement.
  • Support patients in adhering to medical treatments initiated by primary care providers, offering coping strategies for any side effects experienced.
  • Maintain a high standard of ethical and professional conduct in line with the Code of Ethical Conduct for Clinical Professionals.
  • Complete additional tasks as assigned to support the goals and operations of the organization.
Patient Population / Client Focus

As a Licensed Professional Counselor at Peer Support Behavioral Center, you will primarily serve a diverse patient demographic that includes individuals facing varying mental health challenges. Compensation and Benefits

We offer a competitive salary range of USD 87,000 โ€“ 105,000 annually, commensurate with experience.
