Job description

While this role is remote, candidates must reside and licensed in Louisiana or Tennessee. Travel is estimated to be up to 10% and expected schedule requirements are 4x10s 8:00 am - 6:00 pm (Monday-Friday) local time.

What You'll Do  

As an APP with Imagine Pediatrics, you influence and ensure the best clinical care for your patients. Our goal is to increase the number of safe days at home by ensuring kids and families get what they need when they need it-reducing unnecessary medical visits and hospitalizations. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including registered nurses, navigators, dietitians, pharmacists, psychiatrists, social workers, and therapists. You will:

  • Manage a panel of complex pediatric patients - Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination.
  • Work in a multidisciplinary team - Work with pediatricians, nurses, pharmacist, dietitian and social workers to provide wrap-around care in a virtual-first model.
  • Empower families - Educate and support parents to navigate their child's health journey with confidence, ensuring long term health success.
  • Deliver high-quality virtual care - Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team.
  • Ensure continuity of care - Step in when needed to bridge care gaps, coordinating prescriptions, and streamline referrals, to remove barriers to care. 

What You Bring & How You Qualify

  • Minimum 5 years' experience with a pediatric population required, Community Based Health or Complex Care strongly preferred. 
  • You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification.
  • Active LA or TN licensure required
  • Experience and comfort with children with complex medical needs 
  • Experience with Medicaid and/or Community health preferred 
  • Virtual care experience a plus. 
  • Strong desire to improve the quality of life for pediatric patients and their caregivers. 
  • Ability to lead and guide a clinical team. 
  • Bilingual Spanish is strongly preferred.

The compensation for this role starts at $115,000 in addition to annual bonus and competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
