Remote Pediatric Nurse Jobs (NOW HIRING)

Job Overview
The Good Care Helpers is seeking an experienced and dependable Pediatric Registered Nurse (RN) to complete skilled nursing assessments for pediatric patients in their homes on an as-needed (PRN) basis.
This is an assessment-focused position and is not a continuous private-duty nursing or regularly scheduled shift position. The RN will evaluate each patient’s clinical condition, determine skilled nursing needs, develop or update the nursing plan of care, communicate findings to the appropriate healthcare providers, and ensure all required documentation is completed accurately and on time.
Assignments will be offered based on patient referrals, assessment needs, geographic location, and RN availability.
Primary Responsibilities

• Conduct comprehensive in-home skilled nursing assessments for pediatric patients.
• Complete initial assessments, reassessments, recertification assessments, resumption-of-care assessments, and other clinical visits as assigned.
• Evaluate the patient’s medical condition, functional abilities, developmental needs, home environment, safety risks, caregiver support, and need for skilled nursing services.
• Obtain and document vital signs, medication information, diagnoses, allergies, medical history, nutritional status, equipment needs, and current treatments.
• Assess pediatric patients with complex medical needs, including feeding tubes, seizure disorders, respiratory conditions, developmental disabilities, tracheostomies, ventilators, and other specialized healthcare needs, when applicable.
• Develop, review, and update individualized nursing plans of care based on the physician’s orders and the patient’s assessed needs.
• Verify that physician orders, medication profiles, emergency plans, and clinical interventions are complete, accurate, and appropriate.
• Identify changes in condition, safety concerns, unmet needs, or potential barriers to care and promptly report findings to the Clinical Supervisor and ordering provider.
• Communicate and coordinate with physicians, caregivers, therapists, case managers, schools, insurance representatives, and other members of the interdisciplinary care team.
• Provide patient and caregiver education regarding diagnoses, medications, treatments, medical equipment, infection prevention, emergency procedures, and the plan of care.
• Determine whether the home environment can safely support the ordered services.
• Complete all clinical documentation within agency-established timeframes and in accordance with applicable state, payer, accreditation, and HIPAA requirements.
• Participate in case conferences, care coordination, and clinical follow-up when required.
• Follow all agency policies, infection-control standards, professional nursing standards, and applicable laws and regulations.
• Perform additional PRN supervisory or skilled nursing visits when separately assigned and accepted.

Required Qualifications

• Current, unrestricted Registered Nurse license in the state where services are provided.
• Graduate of an accredited registered nursing program.
• Current CPR/BLS certification.
• Minimum of one year of professional nursing experience.
• Pediatric, home health, private-duty nursing, or complex-care experience strongly preferred.
• Knowledge of pediatric assessment, medication reconciliation, care planning, clinical documentation, and caregiver education.
• Ability to recognize changes in condition and determine when provider notification or emergency intervention is required.
• Strong clinical judgment, organizational skills, and attention to detail.
• Ability to complete accurate and timely electronic documentation.
• Excellent written, verbal, and interpersonal communication skills.
• Ability to work independently while maintaining communication with the agency’s clinical leadership.
• Reliable transportation, a valid driver’s license, and current automobile insurance.
• Ability to travel to patient homes within the agency’s service area.
• Ability to successfully complete required background checks, health screenings, competency evaluations, and orientation requirements.

Preferred Qualifications

• Two or more years of pediatric nursing experience.
• Previous experience conducting home-health admissions, skilled nursing assessments, reassessments, or recertification visits.
• Experience caring for medically complex children.
• Familiarity with Medicaid, managed-care, private-duty nursing, or home-health documentation requirements.
• Experience developing and updating pediatric nursing plans of care.
• Experience with electronic medical record or home-health documentation systems.

Schedule and Work Arrangement

• PRN/as-needed position.
• No guaranteed minimum number of visits or hours.
• Visits are assigned based on agency needs, patient location, and RN availability.
• The RN may accept or decline offered assignments based on availability.
• Some assessments may require evening or weekend availability depending on patient and caregiver scheduling needs.
• Travel to patients’ homes is required.

Physical Requirements

• Ability to enter and move safely within patients’ homes.
• Ability to perform pediatric physical assessments and use standard nursing equipment.
• Ability to sit, stand, bend, reach, and lift or reposition equipment as reasonably required during an assessment.
• Ability to respond appropriately to urgent changes in a patient’s condition.
