) Remote Patient Monitoring (RPM) Nurse - Remote
Join Our Growing Clinical Team!
We are seeking compassionate, detail-oriented Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to join our Remote Patient Monitoring (RPM) team. This is a fully remote position where you will monitor patients' vital signs, identify clinical trends, and collaborate with providers to improve patient outcomes.
Responsibilities:
  • Monitor daily patient vital signs through our Remote Patient Monitoring platform.
  • Review blood pressure, pulse, oxygen saturation, weight, blood glucose, temperature, and other biometric data.
  • Identify abnormal trends and escalate clinically significant findings to the appropriate provider.
  • Communicate with patients by phone to assess symptoms, provide education, and encourage adherence to care plans.
  • Document all patient interactions and clinical interventions accurately in the electronic medical record.
  • Collaborate with nurse practitioners, physicians, and the clinical team to ensure timely follow-up.
  • Meet Medicare RPM documentation requirements while delivering high-quality patient care.

Qualifications:
  • Active RN or LPN license in good standing.
  • Strong assessment and critical thinking skills.
  • Excellent verbal and written communication skills.
  • Comfortable using electronic medical records and remote monitoring technology.
  • Ability to work independently while managing multiple patients efficiently.
