• Conduct individual and group therapy sessions, employing evidence-based therapies such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) to assist clients in managing their mental health conditions.
• Work closely with interdisciplinary teams to create, implement, and monitor comprehensive treatment plans designed to address the unique needs of each patient.
• Provide crisis intervention services to support clients in severe emotional distress, ensuring timely referrals to additional resources as necessary.
• Maintain meticulous and confidential patient records in compliance with HIPAA regulations, ensuring that all documentation reflects the comprehensive care provided.
• Engage in regular staff meetings, case conferences, and peer supervision sessions to enhance personal clinical skills and contribute to high-quality patient care.

This position focuses on serving individuals in hospice care, addressing not only their mental health needs but also the psychosocial complexities of serious illnesses. You will interact with a diverse client population, providing support to patients and their families as they navigate the emotional challenges that accompany end-of-life care.

• A Master’s degree in Social Work from an accredited program and valid state licensure to practice as a Clinical Social Worker (LCSW).
• At least 2 years of experience in mental health counseling, ideally within outpatient settings or hospice care environments.
• Proven proficiency in the application of evidence-based therapeutic modalities to deliver effective treatment.
• Strong interpersonal and communication skills with the ability to build rapport with patients and their families effectively.
• Capacity to work autonomously while thriving as a team member in a dynamic healthcare environment.

• Experience in hospice or palliative care settings, understanding the unique challenges faced by patients and families during end-of-life transitions.
• Familiarity with electronic health record (EHR) systems and virtual collaboration tools; knowledge of best practices in documenting patient interactions.
• Involvement in community outreach or program development aimed at improving mental health services for vulnerable populations.
