As part of our community health clinic, you will engage in collaborative efforts with colleagues, participate in regular virtual meetings, and utilize digital tools to support service delivery. • Conduct comprehensive assessments and document the results and treatment plans in compliance with health and legal standards.
• Facilitate psychoeducation and process groups, providing patients and families with essential knowledge and skills to promote mental wellness.
• Collaborate with a multidisciplinary team to evaluate and plan for patient discharges, ensuring effective follow-up and coordination of care.
• Design and implement individualized treatment plans that align with the unique needs and goals of each patient.
• Monitor and assess the safety and environmental needs of patients, responding promptly to any changes in their condition or potential risks.
• Identify and manage high-risk factors—such as suicidal ideation, self-harm, or familial violence—through dedicated screening and intervention strategies.

Your work will primarily focus on healers of mental health, attending to clients from varied backgrounds facing diverse challenges, including anxiety, depression, trauma, and relationship issues. As part of a community health clinic, you will engage with multi-generational families, individuals from underserved populations, and clients exhibiting varying levels of mental health needs, thereby enriching your professional experience while making a meaningful impact.

• Must hold a valid license as an LCSW, LMFT, or LPCC.
• A master's degree in behavioral sciences or a related field from an accredited institution is required.
• Proven ability to complete and submit comprehensive documentation in a timely manner, ensuring compliance and accuracy.
• Exemplary communication skills, both written and verbal, to effectively interact with clients, families, and multidisciplinary team members.
• Demonstrated understanding of cultural competency, comfort, and adaptability when working with diverse populations.
• Must successfully pass a background check.

• Experience working in community health settings or similar environments is highly preferred.
• Previous roles involving group therapy and psychoeducational facilitation will be advantageous.
• Familiarity with remote therapy platforms and digital documentation systems is a plus.
