Job description

You will be a Virtual Care Coach working under the general supervision of physicians or nurses to deliver RPM and/or CCM programs. This is a fully remote, phone- and computer-based position focused on patient outreach and care coordination.

Key Responsibilities

  • Manage a panel of up to 200-300 patients
  • Make 30–60 outbound calls per day to engage and support patients
  • Review health monitoring alerts and escalate as needed
  • Document all interactions using our platform
  • Follow and update care plans
  • Communicate clearly with patients and their caregivers

Required Qualifications

  • Certified/Registered Medical Assistant, LPN, or LVN (must be active and valid in your state)
  • Bilingual: Spanish and English
  • Must reside in the United States
  • Comfortable making high-volume outbound calls and using computer-based tools
  • Excellent phone and written communication skills
  • Quiet, secure remote work setup with reliable internet

MA's and LPN's/LVN's work under 1bios client physician and/or nurse general supervision and utilize the 1bios system to deliver RPM and CCM to patients. This position is a remote position where you will NOT be providing in-person services, you will be a virtual care coach helping people with chronic conditions. Care Coaches will help to onboard and deliver the RPM and/or CCM programs, and help patients manage chronic health conditions, review and triage remote monitoring alerts, collaborate with an extended care team, coordinate care, follow and update patient care plans, and support patients in achieving their health goals.
