Job Description: We are seeking a highly skilled and compassionate Chronic Care Manager Nurse Practitioner or Physician Assistant to join our team. The ideal candidate will have a strong background in chronic care management and patient monitoring. This role requires excellent communication and organizational skills, as well as the ability to delegate tasks effectively to care coordinators. This position offers the flexibility to work remotely.

Key Responsibilities:

  • Provide chronic care management services to patients with chronic conditions.
  • Develop and implement individualized care plans in collaboration with patients, their families, and healthcare providers.
  • Monitor patient progress, assess their needs, and make necessary adjustments to care plans.
