• Utilize your educational background and clinical training to conduct thorough psychosocial assessments and formulate diagnoses, ensuring each patient receives personalized care.
• Collaborate with patients to create individualized treatment plans that align with their values and preferences, focusing on their mental and emotional well-being.
• Implement evidence-based interventions, such as motivational interviewing and problem-solving techniques, to foster engagement and promote positive mental health outcomes.
• Work closely with interdisciplinary teams, including psychologists, physicians, and nutritionists at Aloha Healthcare Center, to provide coordinated care that addresses all aspects of the patient's health.
• Monitor patient progress through systematic tracking of treatment responses, adjusting care plans as necessary to ensure effective support.
• Assist in psychotropic medication management, offering guidance on adherence, monitoring for side effects, and evaluating the overall impact of prescribed treatments.

Your expertise will primarily serve patients facing end-of-life issues, providing essential emotional support and counseling to both the patients and their families. The work demands a high level of empathy, sensitivity, and understanding of the unique challenges associated with hospice care. Your role will be instrumental in helping patients navigate the complexities of their situations while ensuring their dignity and comfort during this critical phase of life.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a current, valid, and unrestricted LCSW license to practice independently in the state of Kansas.
• A minimum of one year of experience in a similar role as an independently licensed social worker.

• Experience in hospice, palliative care, or similar healthcare settings is highly desirable.
• Background in working with mentally distressed patients and a familiarity with psychosocial interventions relevant to end-of-life care.

This full-time position offers a competitive salary range of $80,000 to $100,000 per year, commensurate with experience. In addition to a robust salary, you will be eligible for an annual incentive bonus that rewards your contributions to the organization.
