• As a Licensed Clinical Social Worker, you will evaluate, diagnose, and treat various psychosocial issues, employing clinical expertise to support patients and their families in overcoming personal and environmental obstacles.
• You will adhere to the National Association of Social Workers (NASW) Code of Ethics, maintaining professional standards in your practice.
• Conduct thorough bio-psychosocial evaluations to identify barriers to treatment and enhance patient health outcomes.
• Collaborate with our medical and support staff, providing essential insights into patients' psychosocial needs to foster effective care planning and coordination.
• Develop and implement individualized treatment plans utilizing both individual and group therapeutic modalities, continuously assessing their effectiveness to optimize patient health.
• Document all social work interventions meticulously within patients' medical records, ensuring compliance with departmental standards.
• Provide educational support to patients and their families to facilitate understanding and adherence to treatment recommendations, promoting optimal functioning.
• Advocate for patients by coordinating community resources and support services aimed at enhancing social networks and improving patient outcomes.
• Stay informed about available community resources, utilizing them effectively to support patients’ needs.
• Engage in ongoing professional development activities to enhance your skills in the clinical domain.
• Comply with all departmental policies, procedures, and regulations to maintain a safe and effective care environment.

Your work as an LCSW will primarily focus on patients experiencing a range of mental health challenges affected by psychosocial stressors. This includes individuals facing adjustment disorders, anxiety, depression, and those navigating the complex emotional landscape of chronic illnesses or terminal conditions in a hospice setting. You will be instrumental in offering support to both patients and their families during these critical moments.

• A Master’s Degree in Social Work from an accredited institution, along with a current LCSW license from the State Board of Social Work Licensure.
• Strong understanding of human development, aging processes, and various cultural patterns informing the psychosocial context of care.
• Minimum of one year of clinical experience in a health or mental health setting, particularly with populations similar to those served by our organization.

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with integrated care models and evidence-based therapeutic approaches.
• Proven ability to develop comprehensive care plans that address complex psychosocial needs.
