Description

One of our clients, who delivers home health solutions, in partnership with payers and providers, is seeking remote patient service coordinators to join their growing team. Job Overview: The Patient Service Rep is responsible for reaching out to patients recently discharged from the hospital to schedule an initial assessment call with a Nurse Coach. The Patient Advocate is responsible for meeting and or exceeding weekly patient contact and engagement metrics. Monitors and supports clinicians to ensure service levels and requirements are met. The Patient Advocate will participate in the collection and documentation of all required data for end-to-end seamless care. This data generates reports and analysis to maintain the clinical program's goals. Job responsibilities: โ€ข Makes outbound calls to patients recently discharged from the hospital to schedule an initial assessment with the Nurse Coach. โ€ข Responsible for servicing patients making incoming calls with questions and/or returning messages to be scheduled with Nurse Coaches. โ€ข Understands that timely and accurate documentation is critical to the success of CareCentrix. โ€ข Provides non-clinical issue resolution and escalation of an issue to management when appropriate. โ€ข Participates in and contributes to performance and process improvement activities. โ€ข Must be committed to high quality standards โ€ข Responsible for speaking with physicians and coordinating in-home care for the patient โ€ข Participates and provides feedback when implementing process changes and raises opportunities to leadership ensuring compliance to policies, legal requirements and regulatory mandates. โ€ข Perform other duties as required or assigned. โ€ข Based on business needs, evening and weekend coverage may be needed. A Day in the Role: โ€ข As soon as someone is discharged from the hospital, the team reaches out to help accommodate their needs โ€ข Making outbound calls to patients to schedule nurse assessment (50-100 daily) โ€ข Reaching out to patient, explain the Nurse Coach program. โ€ข Then prepare cases to be sent to the nurse coach after the initial assessment (This includes discharge information, everything the nurse coach will need prior to speaking with patient) โ€ข Making follow-up calls to patients regarding feedback on service โ€ข Working through salesforce platform on โ€œbinsโ€ and completing those calls โ€ข Some โ€œbinsโ€ will have 50 people to call through and others 100; calls vary per day โ€ข Pre-assessment calls could be 3.5-4 minutes on average โ€ข Follow-up calls to check in on patients could be 2-3 minute calls on average โ€ข Everything is provided via script and well put together to ease all situations /calls
