• Conduct comprehensive psychosocial assessments to gauge client mental health status, strengths, and areas for growth.
• Facilitate individual, family, and group therapy sessions using evidence-based interventions that align with clients’ specific needs.
• Utilize therapeutic modalities such as cognitive-behavioral therapy (CBT), motivational interviewing, and family systems therapy to create personalized treatment plans.
• Develop and implement detailed treatment plans that include measurable goals and continuous progress monitoring, ensuring accountability and adaptability.
• Collaborate with an interdisciplinary team, including psychiatrists and nurses, to ensure holistic care and facilitate effective communication among all stakeholders involved in client treatment.
• Provide crisis intervention services, conducting risk assessments and safety plans for clients facing immediate mental health challenges.
• Advocate for clients by connecting them to community resources, offering case management support to enhance their access to necessary services.
• Engage in outreach and community engagement activities to diminish stigma surrounding mental health and improve accessibility to our services.

This role specifically focuses on clients in hospice care, a unique and sensitive population that requires not only mental health support but also emotional and social resources. You will work with individuals and families navigating complex challenges at the end-of-life stage, impacting mental health significantly. Your role will be critical in providing empathy, understanding, and tailored interventions that can help these clients navigate their emotions and experiences.

• A Master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrate strong cultural competence and a commitment to working inclusively with diverse populations.
• Excellent communication, empathy, and problem-solving abilities, specifically in emotional and sensitive contexts.

• Experience in community mental health, hospice care, or similar settings is preferred and will be viewed favorably.
• A background in working with end-of-life issues and providing emotional support to patients and families is highly regarded.

We offer a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
