Are you a detail-oriented Licensed Vocational Nurse who thrives in a fast-paced, collaborative environment? We are looking for a dedicated professional to join our hospice team as a part-time Weekend Clinical Support LVN. Hours are: Friday 5:00pm-9:00pm, Saturday 8:00am-9:00pm, Sunday 8:00am-9:00pm.
In this role, you will be the heartbeat of our branch operations, ensuring our patients receive seamless, high-quality care through expert scheduling, coordination, and clinical administrative support.

As the first and most experienced Hospice Care provider in Texas, VNA has a trusted reputation for delivering high-quality care for over 90 years. What You'll Do
  • Coordinate Care: Schedule field visits, prioritize nursing assignments, and ensure all referrals and admits are processed efficiently.
  • Operational Excellence: Manage the weekend log, run compliance reports, and facilitate DME (Durable Medical Equipment) orders to ensure patients have what they need, when they need it.
  • Clinical Collaboration: Work closely with RNs, Physicians, and the IDT to update patient findings and manage documentation such as DNRs and triplicates.
  • Patient Advocacy: Act as a key point of contact for families, providing information, education, and addressing concerns with empathy and professionalism.

Qualifications, Skills, and Abilities:
  • Education: Graduate of an accredited school of vocational nursing.
  • Licensure: Current State license as an LVN and a valid State Driver's License.
  • Experience: Minimum of two years of experience as an LVN in a clinical setting; hospice or home health experience is strongly preferred.
  • Skills: Strong computer proficiency, excellent interpersonal communication, and top-tier organizational abilities.
  • Requirements: Reliable transportation and current auto liability insurance; must be able to pass a criminal background check.

  • This position involves a hybrid schedule, with duties performed both remotely and in the office.
