• Maintain meticulously accurate and timely clinical documentation, including treatment progress notes and assessments, in adherence to departmental policies.

• Engage with clients' families and support systems to enhance treatment effectiveness and facilitate a smooth transition during discharge planning.
• Create individualized treatment plans that reflect the specific needs, strengths, and circumstances of each patient.
• Conduct regular counseling sessions with clients to provide therapeutic support, making necessary adjustments to treatment plans based on ongoing assessments of patient progress.
• Perform continuous evaluations of patient outcomes and reports to ensure that treatment goals are met effectively and timely.

The ideal candidate will work with a diverse population of clients ranging in age and background, including children and adolescents in school settings, individuals dealing with substance abuse issues, and adults in need of comprehensive mental health support. • A minimum of three years of professional experience in social work or a similar field is required.

• Strong organizational skills and a self-motivated approach to managing workload, documentation, and billing responsibilities are essential.

Familiarity with evidence-based practices in clinical social work and proficiency in using electronic health record systems are advantageous.
