• Engage in regular communication with medical providers to relay updates on patient progress, addressing any issues or concerns that may arise during treatment.
• Document detailed and timely clinical and progress notes, adhering to departmental policies to ensure compliance and continuity of care.
• Collaborate with healthcare team members to craft comprehensive treatment plans based on thorough psychosocial assessments and use evidence-based interventions for effective outcomes.
• Provide direct counseling and crisis intervention, utilizing techniques tailored to each patient’s emotional and behavioral challenges.
• Assist clients with discharge planning and care coordination, facilitating access to resources and support systems within the Erie community and beyond.
• Consult and communicate with family members and supportive systems to enhance client treatment strategies and share discharge planning insights.

In your role as a Licensed Professional Counselor, you will work primarily with a diverse outpatient population encompassing a wide range of ages and presenting mental health issues. These may include anxiety, depression, trauma, and various behavioral disorders. You will be instrumental in guiding clients through their mental health journeys, offering support, coping strategies, and therapeutic interventions designed to meet their unique needs and personal goals.

• A Master’s degree in Professional Counseling or Psychology from an accredited institution is required.
• Current active licensure as a Licensed Professional Counselor (LPC) or an equivalent credential.
• At least 3 years of progressive experience in an outpatient counseling environment.
• Demonstrated proficiency in delivering evidence-based treatment modalities and mental health services is essential.
• Strong ability to work autonomously, maintaining high levels of organization and self-direction.
• Competency in using computer systems and software, especially MS Office, for documentation and communication.

Experience in utilizing telehealth platforms effectively for remote counseling sessions is highly desirable. Familiarity with community resources and an established network within the Erie area that can aid in patient referrals and support services will be advantageous.
