Key Responsibilities
  • Conduct thorough psychosocial assessments to identify client needs, concerns, and goals.
  • Develop and implement tailored treatment plans in collaboration with multi-disciplinary team members, including physicians and nursing staff, to ensure comprehensive care.
  • Provide timely communication regarding patient progress, needs, and any challenges faced, facilitating effective care adjustments when necessary.
  • Maintain accurate and organized clinical documentation, including progress notes, treatment updates, and discharge plans, adhering to health center policies.
  • Offer crisis intervention and therapeutic counseling sessions, addressing emotional and behavioral problems that impede patient progress and well-being.
  • Engage and partner with family members and support systems to create a collective approach towards treatment and discharge planning, ensuring that transitions are smooth and supportive.
Patient Population / Client Focus

This role primarily involves working with clients facing various emotional and behavioral challenges, often within the high-stakes environment of a hospice center. You will be at the forefront of providing crucial support for individuals who are coping with significant life transitions and grief, making empathetic communication and professional expertise vital to effective treatment outcomes.

Qualifications
  • A Master’s degree in Social Work with a focus on counseling and mental health is required.
  • Possession of an active state license as a Licensed Clinical Social Worker (LCSW).
  • A minimum of 3 years of experience in an outpatient or similar healthcare setting is essential.
  • Proficiency in digital tools and software including electronic medical records and MS Office applications.
  • Self-motivated and organized individual with excellent time management skills, able to work independently while thriving in a remote environment.
Preferred Experience

Experience working in hospice care, palliative settings, or with terminally ill patients will be considered an asset. Familiarity with grief counseling techniques and crisis intervention strategies is also highly desirable, as they are integral to our patient care model.

Compensation and Benefits
