Unity Care Services is seeking a passionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote capacity. Given the hospice center setting of our services, you will work with individuals and families navigating some of life's most challenging moments. As a Licensed Clinical Social Worker with Unity Care Services, you will:

• Conduct comprehensive mental health assessments, including psychosocial evaluations and diagnostic interviews, to understand the full scope of each client’s needs.
• Collaboratively develop individualized treatment plans that align with the client's goals, cultural considerations, and specific mental health needs.
• Employ evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), in individual, couple, family, and group therapy settings.
• Provide crisis intervention services and work alongside healthcare professionals to maintain continuity of care for clients experiencing acute distress.
• Maintain detailed and accurate clinical documentation including progress notes and treatment plans, in compliance with clinic policies and regulatory requirements.
• Empower clients and their families through psychoeducation, equipping them with skills and knowledge to manage mental health issues.
• Engage in regular outcome assessments to gauge the effectiveness of treatments, implementing adjustments as necessary.
• Keep abreast of current research and trends in mental health to continuously enhance clinical practice and provide optimal care.
• Uphold ethical standards and ensure the confidentiality of sensitive client information at all times.

You will primarily serve clients within the hospice center environment, where sensitivity and empathy are paramount. The population includes individuals facing terminal illnesses, their families, and those grappling with grief and loss. Your role will involve collaborating closely with clients to navigate complex emotional landscapes and support them through critical decision-making processes.

The ideal candidate will possess:

• A Master's degree in Social Work from an accredited institution.
• An active state license as a Licensed Clinical Social Worker (LCSW).
• Strong interpersonal and communication skills, allowing you to establish rapport with clients from various backgrounds.
• The ability to work both independently and as part of a multidisciplinary team, demonstrating flexibility and open communication.

While not mandatory, the following experiences are beneficial:

• Prior experience in hospice care or working with individuals facing terminal illnesses.
• Proficiency in utilizing various therapeutic modalities and familiarity with tools that aid in psychological assessment.
• Exposure to crisis intervention strategies and previous work in high-stress environments focused on mental health.
