As an LPC at Alameda Health Care, your responsibilities will include:

• Conducting thorough assessments of patients’ mental health needs through structured interviews and standardized tools, ensuring that services are customized to align with individual goals.
• Maintaining open lines of communication with supervisors, direct care staff, family members, and other stakeholders to discuss patients’ progress and adjust treatment plans as necessary.
• Creating and presenting a tentative schedule for appointments, home visits, and client observations, while using an integrated electronic medical record (EMR) system to track patient data.
• Coordinating and executing home visits, along with conducting chart reviews and staff interviews to support comprehensive care.
• Implementing the treatment program effectively, ensuring all interventions meet clinical standards and are executed in collaboration with the interdisciplinary team.
• Participating in regular interdisciplinary team meetings to share insights, review treatment plans, and promote coordinated care for our patients.
• Documenting progress through monthly and quarterly notes that track client development and inform future therapeutic strategies.
• Providing education and training to staff and team members on the implementation of treatment plans and documentation standards to uphold quality assurance practices.
• Providing ongoing follow-up with clients through interviews and home visits, reinforcing therapeutic gains and addressing any challenges encountered.
• Acting as a liaison with the local mental health community to enhance resources and support opportunities for patient development and recovery.

To be considered for this position, candidates must possess:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• A solid understanding of legal and ethical standards in mental health practices, coupled with a commitment to adhering to these principles.
• Exceptional interpersonal and communication skills, with the ability to establish rapport effectively with diverse clients and stakeholders.
• A demonstrated capacity to collaborate within a multi-disciplinary healthcare team.
• Proficiency in using EMR software and related digital tools to enhance patient care and documentation.

While not mandatory, candidates with specialized training or certification in areas such as family therapy, couples counseling, trauma-informed care, or substance use treatment will be highly regarded. Experience working with marginalized populations and an understanding of the social determinants of health are also preferable.
