This position operates in a remote setting, allowing counselors to work from home while maintaining an interactive relationship with the outpatient office. Key Responsibilities

As a Licensed Professional Counselor at Alameda Health Care, you will:

• Conduct thorough assessments of patient needs to develop tailored treatment plans.
• Engage in clear and timely communication with supervisors, healthcare staff, families, and clients regarding patient progress and treatment adjustments.
• Schedule and coordinate appointments, home visits, and follow-up consultations in accordance with treatment plans.
• Deliver in-home observations, chart reviews, and assessments to comprehensively understand clients' situations and needs.
• Participate in interdisciplinary team meetings to discuss cases, share insights, and develop integrated treatment strategies.
• Document monthly and quarterly progress notes to track patient development and outcomes.
• Provide mentorship through training sessions for staff and team members on program delivery and documentation.
• Act as a liaison between clients, their families, and community resources to enhance support and development goals.
• Contribute to the modification and creation of training materials aimed at improving our program offerings.

Patient Population / Client Focus

You will be working with a diverse client population that includes both adults and youth, addressing a range of mental health concerns. Specialization in family therapy, couples counseling, trauma, and substance misuse is particularly beneficial, as these areas represent some of the primary focus points of our patient services.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.
• An active and unrestricted LPC license in the state of Georgia.
• A minimum of two years of counseling experience, ideally within a clinical environment.
• A solid understanding of legal and ethical standards in mental health practice.
• Excellent interpersonal and communication skills, along with the ability to build rapport with a variety of clientele.
• Proven collaboration skills with interdisciplinary teams to enhance patient care and treatment outcomes.
• Familiarity with EMR software and technologies that streamline the documentation and treatment process.

Preferred Experience

While not mandatory, we highly value candidates with specializations in:

• Family and couples therapy.
• Substance misuse rehabilitation.
• Trauma-focused counseling.

Compensation and Benefits
