Key Responsibilities
  • Conduct comprehensive assessments of clients to tailor individualized treatment plans that address their unique mental health needs and challenges.
  • Facilitate clear and ongoing communication between supervisors, direct care staff, families, and other relevant stakeholders regarding client progress, adjusting care strategies as necessary.
  • Develop, share, and maintain a tentative schedule of appointments, home visits, and observations to monitor and enhance client treatment efficiency.
  • Coordinate and conduct home visits as needed, ensuring that in-home observations, chart reviews, and interviews are executed thoroughly.
  • Effectively implement and monitor the treatment program plans, ensuring fidelity to evidence-based practices.
  • Engage in Interdisciplinary Team Meetings and contribute valuable insights regarding client care and treatment collaboration.
  • Document monthly and quarterly progress notes to track developments and evaluate outcomes over time.
  • Offer training sessions for staff and team members, ensuring they are well-versed in program implementation and necessary documentation procedures.
  • Regularly conduct follow-up interviews and home visits to provide ongoing support and resources for clients.
  • Manage and maintain accurate client records, ensuring they are easily accessible to all relevant parties, safeguarding confidentiality and compliance.
  • Act as a liaison with the public and mental health community to foster relationships that support program development and client success.
  • Assist in creating innovative training materials and procedures aimed at enhancing programs for individuals.
