  • Maintain clear communication with supervisors, clients, family members, and other stakeholders to provide updates on progress and adjustments to treatment plans.
  • Schedule and conduct regular appointments, home visits, and observational assessments to monitor client well-being.
  • Coordinate in-home observations, chart reviews, and staff interviews to ensure comprehensive care delivery.
  • Participate actively in interdisciplinary team meetings to collaborate and share insights on client progress and interventions.
  • Ensure that all program plans are implemented effectively, assessing their impact and making necessary adjustments.
  • Document and maintain detailed monthly and quarterly progress notes that are readily accessible to relevant parties.
  • Organize and facilitate training for team members on program implementation, documentation practices, and compliance with standards.
  • Act as a liaison between the public and the mental health community, providing support for community development initiatives.
  • Help develop training materials and programs to enhance therapeutic offerings for clients.
