Your work will be directed towards those receiving end-of-life care, working in tandem with medical professionals to provide holistic support.

• Conduct thorough psychosocial assessments, including in-depth interviews and medical record reviews, to understand each patient’s unique emotional, social, and environmental circumstances that may affect their recovery.
• Offer one-on-one counseling and emotional support to patients and their families, helping them cope with adjustment issues, grief, and emotional challenges related to end-of-life care.
• Collaborate closely with the rehabilitation team to develop personalized treatment plans that holistically incorporate patients' psychosocial requirements, focusing on individualized goals.
• Guide patients and families in accessing essential community resources, such as local support groups, vocational rehabilitation services, and housing assistance, to ensure comprehensive care and support.

Your work will directly impact patients receiving hospice care, many of whom are experiencing significant emotional distress due to terminal illnesses. You will be instrumental in creating a comforting atmosphere that addresses psychosocial needs and encourages open communication among patients, families, and healthcare providers. Your expertise will help patients and their loved ones navigate complex feelings surrounding diagnosis and end-of-life transitions.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is essential.

• Previous experience in a rehabilitation or medical setting is highly preferred, particularly in hospice or palliative care.
• A strong understanding of coping mechanisms, grief processes, and adjustment issues related to severe health conditions is beneficial.
• Demonstrated excellence in communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary team members.

The annual salary for this position ranges from USD 70,000 to 100,000, commensurate with experience, plus a sign-on bonus. We also offer a comprehensive benefits package that includes:
