• Engage in regular communication with physicians and other medical professionals to convey patient progress and discuss any concerns or emerging issues related to their therapeutic journey.
• Maintain clear, accurate, and timely documentation, including clinical and progress notes, to comply with clinical standards and department policies, ensuring all records are up-to-date and accessible.
• Collaborate closely with the healthcare team to develop individualized treatment and discharge plans grounded in thorough psychosocial assessments and evidence-based social work interventions.
• Provide counseling and crisis intervention to patients, meticulously assessing their mental health needs and deploying appropriate treatment methods for emotional and behavioral challenges.
• Assist clients in enhancing their discharge plans, facilitating care coordination efforts, and utilizing resources from various regional services to improve their access to required treatments.
• Consult and coordinate with clients' families or support networks regarding treatment plans, ensuring they are informed and supported throughout the therapeutic process.

As a crucial member of our team, you will primarily engage with diverse adult patients experiencing a wide array of mental health conditions, including anxiety, depression, PTSD, and other mood disorders. You will employ your clinical expertise to evaluate and address these challenges through a client-centered approaches, leveraging evidence-based practices tailored to individual needs. Your ability to relate empathetically and effectively with clients will be essential in this role.

• A Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology is required for this position.
• Current licensure as an LMFT, LPC, LCSW, or its equivalent is essential for delivering therapy services legally and ethically.
• At least 3 years of demonstrated experience in an outpatient mental health setting, showcasing your ability to handle a diverse patient caseload.
• Experience in delivering evidence-based treatments and mental health services, verifying your capacity to provide quality care.
• Strong organizational skills, demonstrating a proven ability to work independently, self-direct, and manage time effectively in a remote setup.
• Proficient computer skills, especially with MS Office Suite and electronic health record systems, to ensure accurate documentation and seamless communication.

• Experience utilizing specific therapeutic methodologies such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT) is highly desirable.
• Previous involvement in developing community outreach initiatives or programs that align with mental health awareness and education would be advantageous.
• Familiarity with telehealth platforms or remote counseling techniques, especially as they relate to engaging clients therapeutically in a virtual environment.
