About the Role:
Skilled Nursing Pharmacy is looking for a dedicated and knowledgeable Pharmacist Consultant LTC to join our growing team in Industry, CA. This is an exciting opportunity to make a meaningful impact on patient care within long-term care settings while working alongside a passionate team of healthcare professionals.
Responsibilities:
  • Conduct monthly medication regimen reviews (MRRs) for long-term care facility residents
  • Collaborate with physicians, nursing staff, and interdisciplinary care teams to optimize drug therapy outcomes
  • Identify and resolve drug-related problems, including interactions, duplications, and contraindications
  • Ensure regulatory compliance with state and federal LTC pharmacy guidelines including OBRA regulations
  • Provide clinical recommendations and pharmacotherapy consultations to facility staff
  • Participate in quality assurance and performance improvement (QAPI) initiatives
  • Assist in developing and updating facility medication policies and procedures

Requirements:
  • Active and unrestricted Pharmacist license in the state of California
  • Doctor of Pharmacy (PharmD) degree or equivalent from an accredited institution
  • Prior experience in long-term care (LTC), geriatric, or consultant pharmacy preferred
  • Strong knowledge of OBRA regulations, CMS guidelines, and LTC compliance standards
  • Excellent communication and interpersonal skills for working with multidisciplinary teams
  • Consultant Pharmacist certification (BCGP or similar) is a plus
  • Highly organized, detail-oriented, and able to manage multiple facility accounts
