As a BHH Systems Navigator, at Sagent Behavioral Health, you will be a part of a community-based team providing care management services for individuals with a mental health diagnosis. Care Managers make a major impact on your local community by helping individuals get connected to mental health services, preventative care, and community programs. BHH is a team-based model that includes a Systems Navigator, BHH RN Supervisor and a Quality Health Home Specialist (QHHS). The Systems Navigator, QHHS, and RN work as a team, and will work with clients on goals related to: connection to mental health services, recovery support services, preventative care, and community programs such as housing, food shelves, employment, transitions in care, etc.

Responsibilities: (no more than 8 bullet points)
  • Maintain a caseload of up to 56 clients.
  • Work closely on a team with a nurse and a QHHS.
  • Ensure that the client is aware of resources and is supported in efforts to access resources in order to address the client's identified goals and needs (e.g., county social services, housing, employment).
  • Assist as needed in conducting the initial needs assessment and developing the preliminary action plan
  • Provide clients with health promotion services by assisting them with skills related to overall health and wellness
  • Interact with clients and/or their identified support on the phone, via video, or in-person at least one time per month.

Requirements: (no more than 5 bullet points)
  • Qualified as a Case Manager or Case Manager Associate per MN State Statute 245.4871 Subdivision 4.
  • Desire to work with adults diagnosed with a mental health diagnosis in an in-home setting.
  • Knowledgeable about community resources and health interventions.
  • Valid Driver's License, Auto Insurance and reliable transportation

Compensation: $52,000- $55,000 annually.
*Actual compensation may be determined by various factors such as education, experience, skillset, internal equity, schedule and/or location.
