PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Conduct telephone and electronic triage assessments using evidence-based clinical protocols and nursing judgment.
  • Assess patient symptoms, determine acuity, and provide appropriate care recommendations.
  • Schedule urgent or same-day appointments when clinically indicated.
  • Provide patient education regarding medications, chronic disease management, preventive care, and home treatment measures.
  • Escalate emergent concerns appropriately and direct patients to emergency services when necessary.
  • Document all patient interactions accurately and timely within the electronic health record (EHR).
  • Collaborate with physicians, advanced practice providers, behavioral health staff, care coordinators, and other interdisciplinary team members.
  • Support continuity of care through referrals, follow-up communication, and care coordination activities.
  • Monitor patient messages, refill requests, laboratory results, and clinical inboxes in accordance with organizational workflows.
  • Obtain prior authorizations for medications, durable medical equipment (DME), and specialty services as required by insurance carriers.
  • Coordinate with pharmacies, insurance companies, and specialty providers to facilitate timely patient care and treatment approvals.
  • Participate in quality improvement initiatives, population health outreach, chronic care management, and patient satisfaction efforts.
  • Maintain compliance with HIPAA, HRSA, Betances Health, state nursing regulations, and organizational policies and procedures.
  • Assist with departmental workflows, administrative support functions, and special projects as assigned.
  • Perform other duties and responsibilities as assigned to support organizational and operational needs.

Requirements
KNOWLEDGE, EDUCATION, SKILL, AND ABILITIES REQUIRED:
  • Current NYS Registered Nurse (RN) license in good standing.
  • 3 years of ambulatory care or community health experience.
  • Strong working knowledge of eClinicalWorks and documentation standards.
  • Experience leading or mentoring clinical support staff.
