• Conduct comprehensive psychosocial assessments to evaluate mental health status, strengths, and needs of patients.
• Utilize evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to meet client needs.
• Collaborate with a multidisciplinary team, including psychiatrists and nurses, to design and implement personalized treatment plans.
• Facilitate individual and group therapy sessions, creating a supportive environment for open discussion and healing.
• Assist patients and their families in understanding diagnoses and available treatment options through education and resources.
• Navigating the complexities of the mental health system, advocate for patients' rights and provide support through the recovery journey.
• Organize and lead psychoeducational sessions and therapeutic groups that focus on skill development and coping strategies.
• Participate in discharge planning meetings to ensure continuity of care and support for patients transitioning out of active treatment.

The Licensed Independent Clinical Social Worker will work with a diverse patient population, including children, adolescents, adults, and families facing various mental health challenges. Our primary focus is to provide tailored support, helping clients navigate issues such as anxiety, depression, trauma, and relational conflicts. • Master's degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

• Proven ability to work collaboratively within a multidisciplinary team, demonstrating exceptional communication skills.

• Experience in community health settings, particularly with underrepresented or high-risk populations.
• Familiarity with electronic health record (EHR) systems for documentation and case management.
• Prior experience in conducting telehealth sessions successfully.
