• Conduct comprehensive client assessments to establish emotional, psychological, and social needs, followed by the formulation of personalized treatment plans.
• Deliver therapeutic interventions including individual, couples, and family therapy sessions, adapting techniques to meet the specific needs of diverse client backgrounds.
• Collaborate closely with interdisciplinary teams, including nurses, physicians, and other healthcare professionals, to provide comprehensive care that addresses both physical and mental health needs.
• Document and maintain meticulous client records and progress notes in adherence to HIPAA regulations, ensuring continuity of care and compliance with legal standards.
• Engage in regular supervision, case consultations, and ongoing professional development activities to enhance knowledge of current therapeutic practices and standards.
• Ensure adherence to ethical guidelines and legal standards within all aspects of clinical practice, including treatment services delivered in a hospice setting.

In this role, you will primarily work with patients in hospice care and their families, many of whom are coping with terminal illnesses and the emotional complexities those situations entail. You will focus on providing grief counseling, end-of-life support, and psychological assessments to help patients navigate their emotions and improve their quality of life during their remaining time. Your ability to create a safe and trusting therapeutic relationship is essential in fostering a positive environment for healing and acceptance.

• A Master’s degree in Social Work from an accredited institution is required, ensuring a solid foundation in social work practices and theories.
• Current licensure as a Clinical Social Worker (LCSW) in Texas, validating your qualifications to practice independently.
• A minimum of 3 years of clinical experience working in social work, preferably with a focus on mental health and/or hospice care.
• Proven knowledge of mental health diagnoses, treatment modalities, and the challenges facing patients in end-of-life care.
• Exceptional communication and interpersonal skills, with an emphasis on empathy and active listening.
• The ability to work independently while also positively contributing to a team-oriented environment within a remote setting.

• Experience in hospice or palliative care settings is strongly preferred, as familiarity with this patient population enhances your efficacy as a provider.
• Experience with electronic health record (EHR) systems to streamline documentation and improve patient care processes.
• Familiarity with grief counseling techniques and practices, enabling you to provide targeted support to patients and families.

Gather and Grow Counseling Center offers a competitive salary range of USD 80,000 – 90,000 per year, commensurate with experience and qualifications.
