The Licensed Clinical Social Worker will be responsible for a range of critical functions:

• Performing comprehensive assessments of clients' psychosocial needs, taking into account emotional, mental, and social factors that influence their overall health and well-being.
• Developing and implementing personalized care plans in collaboration with clients and their families, addressing specific goals and preferences.
• Providing counseling services and emotional support, helping clients navigate the complexities of aging and healthcare transitions.
• Actively advocating for clients’ rights while ensuring access to community resources and appropriate services that meet their needs.
• Maintaining ongoing communication with interdisciplinary teams, participating in case discussions to coordinate care effectively and efficiently.
• Conducting regular check-ins with clients to monitor their progress and address any emerging safety or care issues in a responsive manner.
• Facilitating family meetings to enhance effective communication and resolve conflicts surrounding the care of seniors.
• Documenting all client interactions, assessments, and care plans meticulously in electronic health record (EHR) systems to maintain compliance with regulatory standards.
• Continuing to educate yourself on the latest practices in social work to uphold the highest ethical and professional standards.

Your primary focus will be on seniors and their families who are engaged in hospice care. You will work closely with individuals facing end-of-life challenges, helping them to manage their emotional and psychological concerns while ensuring that their needs are met compassionately and effectively. Your role will involve not just direct client interactions, but also supporting families through this important time, providing them with the reassurance and resources they need.

• Possession of a Master’s Degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, particularly in hospice, geriatric, or community service settings.
• Strong familiarity with social work principles, practices, and aging-related issues, as well as accessible community resources.
• Remarkable assessment, counseling, and interpersonal skills, with a demonstrated ability to connect with seniors and their families.
• Adept in using electronic health record (EHR) systems and other necessary technology tools to streamline documentation and care coordination.

• Experience working in hospice or palliative care settings, with a focus on end-of-life planning and emotional support.
• Familiarity with community outreach programs and resources pertinent to senior care.
• Proven ability to collaborate effectively with healthcare teams and family dynamics.
