Heart & Listen Clinical Counseling is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis. This role is essential in supporting students' emotional, social, and mental health needs within a remote school setting in New Rochelle, New York. The LCSW will work collaboratively with educators, parents, and healthcare professionals to provide holistic, goal-oriented counseling services aimed at fostering students' independence and successful integration into their educational environments.

Key Responsibilities

  • Collaborate closely with interdisciplinary teams, including teachers, school nurses, and other health professionals, to develop and implement tailored treatment plans that address the individual psychosocial needs of each student.

  • Facilitate discharge planning and support transitions to alternative care settings when necessary, ensuring continuity and cohesiveness in care.
  • Monitor and document student progress, adjusting treatment plans as needed based on ongoing assessments and collaborative feedback.
