• Conduct thorough clinical assessments to identify the emotional and psychological needs of patients and their families.
• Develop individualized treatment plans that incorporate evidence-based and solution-focused therapeutic interventions.
• Maintain precise and timely clinical documentation to ensure compliance with healthcare regulations and standards.
• Collaborate closely with a multidisciplinary treatment team, participating in case reviews and contributing insights on patient care.
• Provide education to patients and families about mental health strategies, coping mechanisms, and available resources.
• Stay current with the latest research and developments in mental health and social services to enhance treatment efficacy.

In your role as an LCSW, you will primarily work with patients facing terminal illnesses and their families, addressing emotional distress, grief, and anxiety associated with end-of-life issues. You will facilitate family meetings to support decision-making processes, offer bereavement counseling, and create a safe space for sharing feelings and experiences.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of New York.
• Demonstrated knowledge and proficiency in solution-based psychotherapy practices.
• Strong communication and problem-solving skills, with the ability to navigate complex patient scenarios effectively.

• Previous experience working in hospice or palliative care settings is highly desirable.
• Experience with grief counseling and bereavement support.
• Familiarity with electronic health record systems and clinical documentation tools.
