Job description

Value Based Health company focused on supporting individuals with chronic kidney disease and their comorbid/polychronic conditions is seeking a REMOTE Nurse Practitioner in the  Hendersonville, Nashville, TN area.

Job Details

  • Location: Remote, but candidates should live near Hendersonville,  Nashville, TN.
  • Start Date: ASAP
  • Full Time – No Weekends
  • Negotiable Pay/ Weekly Compensation
  • Duration/Schedule: 3 months with likely extensions
  • Patient Volume: Complete 3+ patient visits daily

Responsibilities

  • Maintain quality of care standards
  • Assist patients with enrolling in educational programs
  • Knowledge of disease diagnosis and prevention
  • Assess and evaluate patients' health status
  • Implement and follow up on care plans
  • Document patient information comprehensively and timely
  • Counsel and educate patients and families about health benefits and programs

Requirements

Experience: 2 years of direct patient care relating to polychronic conditions Polychronic conditions, also known as multiple chronic conditions (MCCs), refer to the presence of three or more chronic diseases in a single patient.
