Telehealth Nurse Practitioner (FNP/AGNP) – Complex Care

Location: Ohio | Primarily Remote
Job Type: Full-Time | 40 hours/week
Salary: $135,000/year

Position Overview

We are seeking an experienced Nurse Practitioner (FNP or AGNP) to join a growing healthcare organization expanding its innovative value-based care model into Ohio.


This is a primarily virtual position focused on managing medically complex adult and geriatric patients through telehealth, remote patient management, proactive clinical intervention, and interdisciplinary care coordination.


Rather than focusing on patient volume, the care model emphasizes identifying patient needs early, preventing avoidable hospitalizations, closing quality gaps, and helping vulnerable patients remain safely and independently at home.


What You'll Do

  • Provide ongoing clinical care through telehealth and remote-care technologies
  • Assess and manage medically complex adult and geriatric patients
  • Manage Medicaid and Dual-Eligible patient populations
  • Develop individualized care plans in collaboration with physicians and interdisciplinary team members
  • Coordinate with patients, caregivers, PCPs, specialists, and health plans
  • Identify clinical changes and intervene before conditions result in unnecessary ED visits or hospitalizations
  • Manage transitions of care following hospitalization or other acute-care episodes
  • Participate in advance-care planning and goals-of-care discussions
  • Participate in shared clinical triage and support
  • Maintain accurate and comprehensive clinical documentation
  • Support HCC coding and risk-adjustment initiatives
  • Address quality gaps and support STAR and HEDIS performance
  • Help develop clinical best practices as the organization's Ohio operations expand


Required Qualifications

  • Master's degree or higher
  • 3+ years of Primary Care experience
  • Active Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) certification
  • Active Ohio NP license or ability to obtain Ohio licensure
  • Appropriate prescriptive authority
  • Experience independently managing adult and/or geriatric patients
  • Strong clinical decision-making skills
  • Strong communication and interdisciplinary collaboration abilities
