In your role as an LCSW, you will engage in a variety of vital tasks and responsibilities, including but not limited to:

• Conducting in-depth clinical assessments to tailor individualized treatment plans that address the unique psychological and emotional needs of patients in hospice care.
• Implementing evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) or solution-focused therapy to foster resilience and address the emotional toll of terminal illness.
• Maintaining secure and accurate clinical documentation that aligns with HIPAA guidelines, ensuring confidentiality and ethical practice at all times.
• Collaborating with a multidisciplinary team of healthcare providers, including physicians, nurses, and specialists, to ensure comprehensive and holistic care is delivered to every patient.
• Participating in regular team meetings and case consultations to contribute insights and enhance strategies for patient support.
• Providing education and support to patients and their families about mental health, coping mechanisms, and resource options available in the community.
• Staying up-to-date with ongoing research and practices within the field of social work and palliative care to continually refine your treatment methodologies.

Your primary focus will be on individuals in hospice care, along with their families, who are navigating the challenges associated with terminal illnesses. This sensitive patient population requires a compassionate approach to addressing their emotional and psychological needs, while also offering practical support during incredibly challenging times. You will help families process grief, impart knowledge about the dying process, and facilitate discussions about end-of-life wishes.

• A Master’s Degree in Social Work from an accredited institution, demonstrating a solid foundation in clinical practice.
• A valid state license as a Licensed Clinical Social Worker (LCSW), indicating your professional credentials and adherence to regulatory standards.
• A minimum of three years of relevant clinical experience, particularly in hospice, palliative care, or a related therapeutic environment.
• An understanding and proficiency in solution-based psychotherapy techniques, allowing for personalized and effective treatment approaches.
• Exceptional communication and interpersonal skills, which are essential for building trust and rapport with both patients and their families.
