As part of the hospice team, you will collaborate virtually with a multidisciplinary team, including medical staff and support personnel. In your role as an LCSW, you will report directly to the Social Work Manager and will be responsible for the following:

• Conducting comprehensive bio-psychosocial assessments to identify psychosocial problems and barriers to treatment, tailoring your approach based on the individual needs of patients and their families.
• Developing and implementing individualized treatment plans, utilizing both individual and group therapies grounded in evidence-based methodologies.
• Providing timely documentation of social work interventions in patients’ medical records to ensure continuity and coordination of care.
• Collaborating with healthcare professionals to deliver holistic care and improve treatment compliance, patient satisfaction, and overall health outcomes.
• Facilitating patient and family education to enhance understanding and adherence to medical recommendations.
• Staying informed on community resources to better advocate for patients' needs and facilitate access to necessary services.
• Maintaining professional knowledge and skills through ongoing training and education, as well as attendance at relevant workshops and conferences.

In this role, you will primarily work with patients in end-of-life care settings, focusing on addressing complex psychosocial issues that impact their well-being. Your ability to empathize with patients and their families, who may be experiencing grief, anxiety, and various socio-environmental challenges, will be critical to their care experience. By fostering open communication and trust, you will help facilitate a supportive environment that encourages patients to express their needs and preferences.

To thrive in this position, candidates must possess the following qualifications:

• A Master’s Degree in Social Work from an accredited institution, along with a current LCSW license from the State Board of Social Work Licensure.
• Demonstrated knowledge of social work practices, especially as they pertain to aging populations, mental health, and hospice care.
• Strong analytical and therapeutic skills necessary for assessing patients and developing effective treatment plans.
• Excellent communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team environment.

A minimum of six months to one year of clinical experience in health or mental health settings is preferred, particularly involving populations served through hospice care. Experience with group therapy techniques and knowledge of healthcare legislation impacting social work practices will be advantageous.
