You will collaborate with a multidisciplinary team of dedicated professionals, participating in team meetings and case reviews via virtual platforms to ensure comprehensive client support.

• Plan, coordinate, and implement personalized support packages that effectively address client needs, helping them manage their difficulties and dependencies.
• Conduct thorough assessments through interviews with service users to gauge their current situation, strengths, weaknesses, and specific requirements.
• Create tailored, measurable goals for each client, ensuring a personalized approach to their treatment.
• Regularly monitor and evaluate client progress, making necessary adjustments to treatment plans based on evolving needs.
• Provide clear information and counseling during sessions, guiding clients on their best pathways forward.
• Maintain precise and confidential records of client interactions and progress to document status and treatment outcomes.
• Participate in case conferences, collaborating with other professionals, and if necessary, providing evidence in court settings.
• Serve as a key worker, consistently cooperating with multidisciplinary teams to ensure holistic care for clients.
• Facilitate referrals to appropriate community services to support clients in their recovery processes.

• A valid LCSW license in Illinois is required.
• A bachelor’s degree in social work (BSW), psychology, or sociology is necessary; a Master’s degree is preferred.
• Demonstrated experience as a Social Worker with a strong understanding of corresponding social theories and practices.
• Exceptional social perceptiveness and empathy, enabling a genuine connection with clients from diverse backgrounds.
• Strong skills in building and maintaining professional helping relationships with clients and colleagues alike.
• Ability to assess complex situations with resilience, offering thoughtful insights and support to clients in need.
• Willingness to undergo background checks as part of the hiring process.

• Previous experience in a community health setting is highly desirable.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
