Key Responsibilities
  • Coordinate and document transitions for patients moving from hospital settings to outpatient care, facilitating smooth discharges in collaboration with hospital staff.
  • Utilize crisis de-escalation techniques effectively while serving as the Clinician of the Day, providing crucial milieu support to all patients.
  • Perform comprehensive risk assessments and create safety plans for individuals at risk of crisis interactions, ensuring each assessment is documented the same day.
  • Report any critical incidents, completing necessary forms within 24 hours to uphold safety and accountability.
  • Carry an after-hours phone on an alternating basis to provide care coordination support, bridging the gap for clients needing immediate assistance.
  • Work proactively with program participants to minimize reliance on crisis services through targeted interventions and support.
  • Maintain crisis alerts for collaboration with county crisis services to optimize service delivery for all participants.
  • Complete additional duties as requested, adapting to the evolving needs of our patients and organization.
Patient Population / Client Focus

Your practice will involve working with diverse populations, including adolescents and adults experiencing a range of mental health challenges. You will be instrumental in addressing urgent needs and providing continuous support that promotes healing and personal growth within the community.

Qualifications
  • A masterโ€™s or doctorate degree in clinical psychology, counseling, clinical social work, or a related field (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
  • An independent license in the state of Oregon as an LCSW, LMFT, or LPC, valid in the USA.
  • Demonstrable expertise in mental health counseling, requiring extensive problem-solving skills, educated judgment, and empathetic communication.
Preferred Experience
  • Proficiency in managing high caseload volumes, with the ability to prioritize tasks effectively and delegate responsibilities when necessary.
  • A track record of motivating clients and teams, demonstrating consistent enthusiasm and positivity in challenging situations.
