Crossbend Behavioral Center PC is seeking a compassionate and dedicated Remote Licensed Professional Counselor (LPC) to join our dynamic mental health team. • Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments to determine initial treatment plans.
• Monitor and ensure that patients receive timely follow-up care, ensuring they are progressing in their treatment and adjusting therapeutic approaches as needed.
• Provide brief, focused interventions and therapy sessions to clients, tailored to their unique circumstances and treatment goals.
• Collaborate with primary care providers to recognize and address mental health and psychosocial issues, offering expert recommendations that may involve referrals to specialty services when necessary.
• Develop and implement relapse prevention plans for patients at risk of deterioration in their mental health.
• Document patients' progress and treatment plans meticulously within the clinic's electronic health record system, maintaining accurate clinical records.
• Educate patients on self-management strategies and coping mechanisms to navigate their mental health challenges effectively.
• Engage with clinic management and multidisciplinary teams to improve behavioral health services and assess outcomes through program evaluations.
• Support patients' adherence to any medical treatment initiated by the primary care provider, addressing challenges such as medication side effects.

The LPC will serve a diverse array of clients, including individuals experiencing anxiety, depression, trauma, and other mental health concerns. Our patient population includes adults, families, and children, reflecting the community's demographics. The LPC will ensure culturally competent care that respects and acknowledges the various backgrounds and personal experiences of clients.

• A minimum of a Master's Degree in Clinical Counseling or a closely related field.
• Current and valid Licensed Professional Counselor (LPC) License in Texas.
• Strong understanding of mental health assessment techniques, treatment planning, and various therapeutic methodologies relevant to different age groups.
• Ability to effectively communicate and establish rapport with patients, their families, and colleagues across diverse cultural backgrounds.
• Experience with electronic health records to maintain clinical documentation efficiently and accurately.

• Bilingual proficiency in English and Spanish is highly desirable, enhancing communication with a broader patient demographic.
• Prior experience in a community health setting or working within integrated care models.
• Familiarity with the guidelines and protocols set forth by Federally Qualified Health Centers (FQHC).

We offer a competitive salary range of $87,000 to $105,000 annually, commensurate with experience and qualifications. In addition to a comprehensive healthcare plan, employees will receive a generous sign-on bonus upon hire.
