  • Coordinate continuity of care by liaising with referral sources and outpatient providers upon patient admission, at midpoint, and during discharge. Confirm all discharge appointments with clients and their families.
  • Initiate discharge planning as soon as the admission occurs, conducting discharge sessions to confirm coping skills, transition plans, and available resources.
  • Manage crises effectively during therapy sessions while providing support to milieu staff in their engagements with clients and families, fostering a therapeutic environment.
  • Ensure timely completion of clinical documentation and utilize electronic records to maintain accuracy and compliance.
  • Establish and nurture therapeutic rapport with clients, conducting thorough assessments for suicidality and potential risks to self or others.
  • Lead admission assessments with professionalism and a welcoming approach, and actively participate in clinical outreach efforts, such as program tours.
  • Engage in weekly treatment team meetings to share progress and coordinate care, contributing to the collaborative effort of providing exemplary treatment.
  • Perform utilization reviews with insurance providers, delivering necessary clinical information to secure initial and continued authorizations.
