• Conduct initial patient intakes, focusing on diagnostic assessments and functional evaluations to inform treatment plans.
• Provide targeted therapeutic interventions for patients in need of mental health services, ensuring adherence to established agency and Federally Qualified Health Center (FQHC) guidelines.
• Assess patients' mental health needs and make informed recommendations to primary care providers regarding treatment approaches.
• Follow up with patients to monitor their progress and adjust treatment plans as necessary, carrying out relapse prevention planning where indicated.
• Develop collaborative plans with a primary care team to address behavioral health needs effectively.
• Document patient progress and interaction thoroughly in clinical records while keeping primary care providers informed.
• Assist patients in complying with any medical treatments initiated by primary care providers.
• Maintain ethical standards in practice by adhering to the Code of Ethical Conduct for Clinical Professionals.
• Perform additional duties as assigned to support team and patient needs.

• A Master’s Degree in Clinical Counseling or a related field is required.
• Current LPC (Licensed Professional Counselor) License is mandatory for this position.
• Proficiency in mental health assessment, treatment planning, and cognitive-behavioral therapies.
• Ability to work effectively with diverse populations, with a thorough understanding of the social and community resources available.
• Excellent documentation and communication skills to maintain patient records and collaborate with healthcare teams.

• Experience working in an outpatient or interdisciplinary healthcare setting.
• Knowledge of psychotherapeutic techniques and their application in unique client scenarios.
• Bilingual proficiency in English and Spanish is preferred to better serve our diverse populations.
