Key Responsibilities

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial concerns, contributing to holistic care for patients.
• Conduct thorough patient intakes that focus on diagnostic evaluations and functional assessments to tailor effective treatment plans.
• Deliver timely emergency mental health interventions to patients in crises, ensuring their immediate needs are addressed.
• Advise primary care practitioners on the suitability of patients for primary care versus specialized mental health services, providing necessary referrals when appropriate.
• Perform regular follow-up evaluations to track progress and adjust treatment strategies based on patient feedback and outcomes.
• Work collaboratively with patients to establish personalized relapse prevention strategies, guiding them in maintaining their mental health stability.
• Identify patients at risk of deteriorating mental health and participate in the development of proactive preventative programs tailored to their needs.
• Supervise the execution of mental health care services, ensuring coordination with both internal teams and external providers as per patient consent.
• Maintain up-to-date documentation of patient records, progress notes, and diagnostic information to ensure continuity of care.
• Regularly communicate with primary care providers regarding patient needs and treatment progress, actively participating in multidisciplinary treatment planning.
• Help patients adopt healthier lifestyle choices to address issues stemming from mental health disorders that impact physical well-being.
• Equip patients with essential self-management skills and educational resources, empowering them to take ownership of their mental health journeys.
• Consult with clinic management and assist in identifying opportunities for program development and evaluation for behavioral services.
• Support patients in adhering to prescribed medical therapies and managing any associated side effects with practical strategies.
• Uphold the Clinical Professionals' Code of Ethics and perform additional responsibilities as assigned.
