• Perform comprehensive patient intakes, focusing on diagnostic and functional evaluations to develop personalized therapy goals and treatment strategies.
• Collaborate with primary care practitioners to identify and address mental health and psychosocial issues, enhancing the effectiveness of the primary care team.
• Provide immediate mental health interventions for clients requiring emergency support, ensuring they receive prompt and focused care.
• Conduct follow-up evaluations to monitor patient progress, adjust treatment plans, and enhance efficacy based on documented outcomes.
• Assist patients in creating and implementing relapse prevention plans, utilizing evidence-based techniques to promote recovery and stability.
• Maintain accurate and thorough records of patient progress and diagnostic information, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) standards.
• Stay engaged with primary care providers to keep them informed of patient needs and treatment progress, facilitating collaborative treatment planning.
• Guide clients in self-management strategies and provide educational resources to encourage proactive involvement in their health journeys.
• Work with clinic management and team members to identify areas for improvement in behavioral services, focusing on outcome evaluations and program development.
• Uphold the ethical standards set by the Clinical Professionals' Code of Ethics while performing additional duties as assigned.

In this role, you will work with a varied patient demographic, including individuals facing mental health challenges, those seeking support for psychosocial issues, and clients requiring assistance with managing physical ailments related to mental health. Your role is crucial in providing empathetic support and effective intervention strategies tailored to each patient’s unique background and circumstances.

• A Master’s Degree in Clinical Counseling is required for this position.
• A valid Licensed Professional Counselor (LPC) license is mandatory.
• Extensive knowledge of mental health evaluation methods, treatment planning, and behavioral health systems is essential.
• Familiarity with DSM-IV diagnoses and various psychotherapy modalities is required.
• Proficiency in both English and Spanish is highly desirable but not mandatory.
• Must possess sufficient mental and physical health to handle the demands of the role.
• A deep understanding of the social implications of mental and physical health issues on individuals and communities is essential.
• Comprehensive knowledge of available community resources is beneficial for assisting clients in accessing necessary support.

Prior experience in a clinical counseling setting, particularly within a telehealth framework, is advantageous. Familiarity with community mental health services and demonstrated success in managing diverse client needs will be considered valuable assets in your application.
