• Perform thorough assessments and diagnostic evaluations using established clinical tools, such as the Diagnostic and Statistical Manual of Mental Disorders, to develop individualized care plans that respect the cultural diversity and specific needs of the patient population.
• Continually monitor and evaluate patient progress, providing tailored interventions that support mental health diagnosis and promote stability.
• Maintain accurate and timely record-keeping for all clinical interactions using technology that aligns with regulatory standards, ensuring compliance and transparency.
• Collaborate with patients and their families to devise comprehensive treatment plans that articulate clear goals and desired outcomes, ensuring their active involvement in the care process.
• Actively participate in interdisciplinary case reviews, contributing to the development and articulation of treatment objectives aligned with organizational goals.
• Connect patients with appropriate resources and services, facilitating smooth referrals and establishing effective communication with other healthcare providers.
• Mentor aspiring mental health practitioners, providing guidance and education to students, interns, and peers, and fostering a culture of learning and excellence within the organization.

• A Master's Degree in Social Work is required.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Massachusetts.

• Proficient in utilizing electronic health record (EHR) systems for documentation and communication.

• Experience working in hospice or palliative care settings is highly desirable.

Sarasota Memorial Medical Center offers an attractive salary range of USD 90,000 – 105,000 per year, commensurate with experience.
