As a remote team member based in Memphis, Tennessee, you will work within an outpatient office setting that fosters collaboration and support. • Conduct thorough assessments on new and existing patients to accurately identify their mental health needs and create appropriate treatment plans.
• Facilitate communication between supervisors, direct care staff, family members, and other stakeholders to update them on patients' progress and challenges.
• Draft and present a tentative schedule to supervisors for appointments, home visits, and monitoring activities to ensure consistency in patient care.
• Coordinate and conduct in-home visits, chart reviews, and staff interviews as part of ongoing patient evaluations and progress monitoring.
• Ensure that all prescribed treatment plans are effectively executed and adjusted as necessary, adapting strategies to meet evolving patient needs.
• Engage in interdisciplinary team meetings to discuss patient cases, share insights, and contribute to comprehensive care strategies.
• Maintain accurate and up-to-date documentation through monthly and quarterly progress notes to track patient outcomes and interventions.
• Design and facilitate training sessions to enhance staff competencies in implementing treatment plans and maintaining accurate documentation.
• Perform follow-up interviews and home visits to monitor patient development and provide additional support as needed.
• Act as a liaison between the organization and the broader mental health community to promote resource access and inform community outreach efforts.

In this role, you will work with a diverse range of clients, including children, adolescents, and adults facing various mental health issues. Your expertise will help individuals process trauma, address concerns related to substance misuse, or navigate complex family dynamics. This multifaceted approach allows for meaningful and impactful therapy tailored to each patient's unique journey.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential for this role.
• Active and unrestricted state licensure as a Licensed Professional Counselor (LPC) is required.
• A minimum of two years of experience in a counseling capacity, preferably in a clinical setting, is necessary.
• A strong background or specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly advantageous.
• Comprehensive knowledge of legal, ethical, and confidentiality standards in mental health practice.
• Outstanding interpersonal and communication skills, fostering connections and rapport with clients from diverse backgrounds.
• Collaborative spirit with a demonstrated ability to work effectively within a healthcare team.
• Familiarity with electronic medical record (EMR) systems and related technologies is necessary for efficient documentation and patient management.

While a minimum of two years of experience is required, candidates with additional experience or specialized training in any of the following areas will be given preference:

• Trauma-informed care practices
• Integrated behavioral health
• Substance use and abuse treatment
• Family systems therapy
