This position allows for a remote work setting, providing flexibility in your schedule while maintaining a strong connection to clients and their families. You will collaborate closely with a multidisciplinary team of healthcare professionals, including nurses, caregivers, and other social workers, all committed to delivering comprehensive and coordinated care. The work hours are primarily Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend tasks based on client needs.

• Conduct comprehensive assessments of clients' psychosocial needs and develop individualized care plans that reflect their goals, preferences, and cultural values.
• Provide empathetic counseling and emotional support to seniors and their families as they navigate the complexities of end-of-life care, addressing their emotional and psychological needs.
• Advocate for clients within the healthcare system, ensuring access to necessary services and resources, including community support programs, therapy options, and financial assistance.
• Engage in regular home visits to monitor the well-being of clients, assess their living conditions, and identify any immediate concerns that may affect their quality of life.
• Collaborate with an interdisciplinary team to ensure that all aspects of clients' care are considered, facilitating effective communication and conflict resolution among family members involved in the care process.
• Maintain detailed and accurate documentation of client assessments, treatment plans, progress notes, and any interactions, adhering to all regulatory requirements and ethical standards.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work role, with a preference for experience in hospice, geriatric, or community-centric settings.
• Extensive knowledge of social work principles, particularly concerning aging, end-of-life issues, and available community resources.
• Demonstrated ability to provide compassionate care, build rapport, and empathize with seniors and their families.
• Strong communication and interpersonal skills, promoting effective collaboration within interdisciplinary teams.
• Familiarity with electronic health record (EHR) systems and proficiency in relevant computer applications.
