Key Responsibilities

• Conduct in-depth assessments to evaluate the emotional, social, and psychological needs of patients and their families.
• Develop tailored, person-centered treatment plans that address specific challenges associated with terminal illness and end-of-life care.
• Implement effective therapeutic interventions, such as solution-focused therapy and grief counseling, to support patients in managing their emotions during this critical time.
• Maintain detailed and confidential documentation of client interactions, assessments, and treatment progress in our integrated care platform.
• Collaborate with physicians, nurses, and chaplains to ensure a comprehensive approach to care that encompasses physical, emotional, and spiritual well-being.
• Stay informed on the latest research and best practices in social work, especially within hospice and palliative care, to enhance your contributions to the team.

Patient Population / Client Focus

You will be working primarily with patients who are experiencing terminal illnesses, alongside their families. Your role will directly impact individuals coping with trauma, anxiety, depression, and grief while providing them with the tools and strategies necessary for emotional support and coping mechanisms during their hospice journey.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Active Licensed Independent Clinical Social Worker (LICSW) licensure in the state of Vermont.
• Strong interpersonal and communication skills, with the ability to build rapport with clients from diverse backgrounds.
• A collaborative mindset with a commitment to working within a multidisciplinary team framework.

Preferred Experience

• Previous experience in hospice, palliative care, or a related field is highly preferred.
• Proficiency in working with telehealth and electronic health record systems.

Compensation and Benefits

We offer a competitive yearly salary ranging from $80,000 to $100,000, commensurate with experience and qualifications.
