POSITION SUMMARY:
In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our CCM, RPM, and BHI services.
KEY RESPONSIBILITIES:
Responsible for establishing the Tapestry Health patient relationship for our Chronic Care Management, Remote Patient Monitoring, and Behavioral Health services.
Be available at a minimum of 10 weekdays per month to conduct approximately 300-400 non-clinical judgment consults onsite and remotely.
These are brief, non-clinical judgment consults, simply advising patients of the services they will get with Tapestry Health. No prescribing or treating involved.

Document patient encounters accurately and thoroughly within our platform at time of visit.
Provide end of day report to program administrators to track progress for completion of visits.
MINIMUM REQUIREMENTS:
Qualified MD/DO
Successful completion of US Residency in Internal or Family Medicine.

Active, unrestricted, Medical Licenses in at least 5 states.
Excellent clinical judgment and decision-making skills.
Must have reliable transportation to travel to assigned buildings.
Commitment to providing patient centered care and improving health outcomes.
Familiarity with electronic health record (EHR) systems and healthcare technology.
Ability to travel within 1-2 hours of assigned region.
COMPENSATION STRUCTURE: 10-25k/month, mileage reimbursement, Malpractice.
