• Conduct thorough psycho-social assessments to evaluate the mental health needs of each patient, formulating personalized care plans to guide treatment.
• Diagnose and treat a range of mental, emotional, and behavioral disorders, regularly modifying care plans in response to patient progress or setbacks.
• Collaborate with a multidisciplinary care team to create effective discharge plans, coordinating with healthcare providers, community organizations, and family members to ensure comprehensive patient care.
• Provide timely referrals for additional services, such as support groups, psychiatrists, or community resources, to enhance patient well-being.
• Maintain compliance with relevant state and federal regulations, adhering to the standards set forth by the Mental Health Practice Act and organizational policies.
• Utilize our secure, HIPAA-compliant digital platform to manage patient records, document progress, and track treatment interventions efficiently.
• Engage in educational and supportive sessions for patients and families, providing them with the necessary information and tools to navigate the mental health landscape.
• Advocate for patients' needs, ensuring effective communication between them and the healthcare system, as well as identifying potential roadblocks in their care journey.
• Participate in professional development activities and contribute to team meetings, sharing insights and strategies to enhance the quality of care.

As a Licensed Mental Health Counselor at Rocky Meadow Counseling, you will work primarily with individuals experiencing a variety of mental health challenges. This includes, but is not limited to, anxiety disorders, depression, trauma-related issues, relationship challenges, and stress management. You will empower clients through evidence-based therapeutic methods while maintaining a focus on holistic well-being and recovery.

• Master's degree in Social Work, Counseling, or a related field from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Psychologist.
• Familiarity with local community mental health resources to enhance patient support and referrals.
• Excellent written and verbal communication skills, essential for building rapport and fostering therapeutic relationships with clients.
• Ability to collect and interpret client data, ensuring responsive adjustments in treatment plans.

• Experience in an outpatient mental health setting is preferred, showcasing an understanding of the nuances of this type of care.
• Diverse experience working with varied client populations, particularly in crisis intervention or trauma-informed care.
• Proficiency in using electronic health record (EHR) systems for case management and document tracking.
