We are seeking a compassionate, proactive, and experienced Mental Health Monitoring Specialist to help launch and grow our behavioral health support services. This individual will play a key role in developing workflows, conducting mental health assessments, coordinating resources, and supporting the emotional well-being of our primarily geriatric patient population.
Position Summary
The Mental Health Monitoring Specialist will provide remote psychosocial and behavioral health support to patients enrolled in TeleMate Health's monitoring programs. This role is ideal for a licensed behavioral health professional, social worker, counselor, or nurse with significant geriatric mental health experience who enjoys building processes, collaborating with interdisciplinary teams, and making a meaningful impact in patients' lives.
As this department continues to grow, this individual will have the opportunity to help shape clinical workflows, assessment protocols, and follow-up processes for behavioral health monitoring.
Key Responsibilities
- Conduct psychosocial and behavioral health assessments to identify patients' emotional, cognitive, and social needs.
- Complete routine mental health evaluations and follow-up assessments as clinically appropriate.
- Develop individualized care plans that address behavioral health concerns and social determinants of health.
- Identify patients experiencing depression, anxiety, grief, isolation, cognitive decline, or other behavioral health concerns and provide appropriate interventions.
- Connect patients with community resources, counseling services, psychiatric providers, crisis resources, and other behavioral health specialists as needed.
- Coordinate care with physicians, nurses, caregivers, and community agencies to ensure continuity of care.
- Educate patients and caregivers on available behavioral health resources and strategies to improve overall well-being.
- Serve as a patient advocate while promoting patient-centered care.
- Accurately document all patient interactions, assessments, and interventions within the electronic health record (EHR).
- Monitor patient progress and provide ongoing follow-up to ensure behavioral health needs are addressed.
- Escalate urgent mental health concerns according to established clinical protocols.
- Assist in creating and refining workflows, policies, and procedures as the Mental Health Monitoring program expands.
- Participate in interdisciplinary team meetings and quality improvement initiatives.
Qualifications
- Current, active professional licensure in one of the following (or similar):
- Bachelors in Social Worker (BSW)
- Masters in Social Worker (MSW)
- Licensed Professional Nurse (LPN)
- Registered Nurse (RN)
- Other comparable behavioral health licensure
- Minimum of two years of experience working with geriatric behavioral health, chronic disease management, or older adult populations preferred.
- Strong understanding of behavioral health conditions, community resources, and social determinants of health.
- Excellent communication, assessment, and organizational skills.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Comfortable creating new workflows and implementing process improvements.
- Experience with electronic health records (EHR) and telehealth platforms preferred.
- High-speed internet connection and a private, HIPAA-compliant workspace.
- Fluency in English required; additional languages are a plus.
- Must reside in the United States and be legally authorized to work for any employer.
